Important to work within the networks of primary care, satellite centers and rehabilitation centers
Patient with moderate lung function that need a simple physical activity programme. These patients are treated in the satellite centers (GP) or in primary care for reconditioning. If these simple patients are diagnosed in rehabilitation centers they are referred to satellite or primary care
Patients with worse lung function needs a more multidisciplinary rehabilitation and need more than an exercise training programme. A patient in this case can be referred from primary care to a more specialized center with multiple disciplines acessible. The patient is referred back to primary care for the maintenance stage.
